police candidates who plan to take a C.H.I.P. Physical
Ability Assessment must read and complete all required
information contained in the C.H.I.P. General Information
Packet. A completed registration form, medical release
form and $65.00 fee must be returned to C.H.I.P. by the
registration deadline. If you have questions regarding
