To Register
To
register for a C.H.I.P. Physical Ability Assessment,
applicants must read and complete a General
Information Packet, including medical approval form and
registration. The following forms, along with the
registration fee, must be submitted to C.H.I.P. prior to the
registration deadline for the corresponding assessment.
REGISTRATION
FORM: Applicants must complete the registration
form included in the general information packet, including
the date of assessment.
MEDICAL APPROVAL FORM: A licensed physician must review the four elements of the Connecticut Police Officer Standards and Training Council’s Physical Fitness Test and determine the candidates ability to safely perform the
Physical Ability Assessment. A signed Medical Approval Form, as included in your General Information Packet, must be presented on the day of testing.
REGISTRATION FEE: Payment of
$65.00 is due
prior to the registration deadline. Money orders, cashiers checks and cash are acceptable forms of payment.
Personal checks and credit cards are not accepted. No refunds will be provided.

On
the Day of the Assessment
IDENTIFICATION: In order to participate in a
Physical Ability Assessment proctored by C.H.I.P., you must provide two forms of identification. One form of identification must be a photo ID. Accepted photo identifications include a valid driver’s license, Connecticut photo identification, military photo identification, or current passport. One other form of identification must also be presented on the day of the assessment.
APPROPRIATE ATTIRE: Appropriate athletic attire must be worn to the assessment including athletic shoes. Weather conditions should be taken into consideration when dressing for the assessment as you will be outdoors during portions of the assessment.
